The application packet is due no later than the last Monday in October. No exceptions. When completed,
hand your application to the NJHS Advisor or to the front office in Building A. Check to make sure that you have
completed and included all parts of your application. Teacher recommendations are submitted separately.
Please assemble your packet in the following order. Make sure your name is on every page! Staple or paper clip
all pages together, so none are lost.
1. Application/nomination packet cover page with signatures at bottom of page
2. Membership information page with acknowledgment signed
3. Student’s Self-Recommendation Letter
4. Student activity information sheet
The Teacher Recommendation form will be submitted by each respective teacher, not the student. It will not
be part of the packet that students turn in.
